Nutrition Counseling is under CPT codes 97802 (initial appointment) and 97803 (follow-ups).

Asking your insurance provider the following can help clarify things further:

• What percentage does your plan cover?  

• What is your deductible?  Is it satisfied?  Is nutrition counseling subject to your deductible?

• What is your co-pay?

• Are there any limits to your coverage?  (specific diagnoses or number of visits)

• Is pre-authorization or a referral needed?
